Attorney Notes - Case File 2026-089

Client Meeting: January 14, 2026

Met with Elena Rodriguez and Mark Rodriguez at our downtown office. They are looking to finalize the independent adoption of their young relative, Leo Mateo. The child is currently three years old, born in San Francisco on August 12, 2022. The child has been living with the couple in their home at 452 Oak Grove Lane, Apt 4B, San Mateo, CA 94403 since September 2024. Elena and Mark are both over 30 years old and are fully committed to providing a stable, loving environment for Leo. They confirmed that the biological parents have signed all necessary relinquishment papers, and there are no legal guardians involved.

We discussed the filing location; since the child lives in San Mateo County, we will be filing the ADOPT-200 in the Superior Court of California, County of San Mateo. The couple is working with an independent adoption facilitator to ensure all state requirements are handled. They mentioned their desire to legally change the child’s name to Leo Mateo-Rodriguez upon approval. I have verified their identities and they are prepared to sign the declarations under penalty of perjury. We also reviewed the ICWA inquiry requirements and confirmed that the child does not have Native American heritage, so we will be attaching the ICWA-010(A) form as a matter of routine protocol. Everything seems in order to proceed with the filing next week.
