Patient Intake Note: Dr. Sarah Jenkins, 14 February 2026. Patient: Mr. Robert Alan Smith (DOB: 12/05/1972). Caretaker: Ms. Linda Jane Smith (DOB: 22/08/1975). Contact: (03) 9876 5432. Robert has been diagnosed with advanced Multiple Sclerosis, his primary condition. He is not currently hospitalized. Linda provides daily care, starting from 15/01/2026. Robert experiences significant physical challenges; he is often able to move around the house with the help of one person but requires a lot of help to move from his wheelchair to a bed. His mobility is not entirely lost, but he definitely cannot do it alone. Showering is a major struggle; he requires a lot of help to bathe, and dressing is similarly difficult, necessitating significant physical support. While he understands most of what I say, he struggles with memory of daily events, often forgetting what occurred earlier in the afternoon. He is generally aware of his location but occasionally expresses frustration through shouting when he feels overwhelmed. He never wanders away from home. He currently takes daily medication for spasticity, which he administers with some help from Linda to ensure correct dosage. He does not have a terminal illness and is expected to live well beyond the next three months. Linda remains his primary support during these periods of difficulty.
