Hey Sarah, I hope your week is going well. I’ve been trying to get our financial paperwork in order for the upcoming tax season, and I realized I cannot find the original title for our 2021 silver Toyota Camry anywhere. I checked the safe, the desk, and even the glove box, but it seems to be completely missing. Since we are planning to sell the car soon, we really need to get a replacement title from the Wisconsin DMV.

I’ve already looked up the process, and we need to fill out the Form MV2119. I have all the details ready: the VIN is 4T1B11HK2MU058291, and the current license plate is WISC-772. The car is still registered here in Dane County, specifically in the Town of Madison. My birthday is 05/12/1985, and I’ll use my Wisconsin driver license number, which is W123-4567-8901-23. You can be the co-owner on the form since we’ve always held the car with an 'OR' relationship so that either of us can handle the title transfer if needed.

Our current mailing address is 1422 Maplewood Drive, Madison, WI 53711. Could you please double-check the form before we send it in? I think we need to include a $20 check for the replacement fee. I'm busy with the renovation project today, so let me know if you need any other info from me to get this mailed off to the DOT office in Madison.
