Enter the proposed date on which the ship is to be registered with the registry.

Enter the name of the port where the ship will be located at the time of registration.

Check this box if the ship has an IM/IMO number and you will provide that number in the adjacent field.

Enter the country under whose laws the ship was last documented and the vessel's current name as shown on that documentation.
