Caseworker Field Visit Summary: Date 05/12/2026. Met with Jordan Taylor today at the downtown community center. We sat in a private room to complete the youth survey. I, Sarah Jenkins, serve as the Regional Placement Specialist for the district, and I walked Jordan through each question. Jordan is doing well—currently, they are employed at a local bookstore working 25 hours a week, but they are not in a full-time position yet. They mentioned that they participated in a retail management internship last summer which really helped them gain confidence. Regarding school, Jordan is fully enrolled in the local community college and is utilizing a Pell Grant to cover their tuition costs. We discussed their financial situation; aside from their paycheck, they receive a modest stipend from a local education foundation. Jordan confirmed they have a supportive mentor, Mr. Henderson, who they reach out to whenever they need career advice. Regarding health, Jordan is currently on Medicaid and that plan provides coverage for their medical, mental health, and necessary prescription drug needs. Jordan reported no history of homelessness and has not been referred for any drug or alcohol counseling services. When asked about legal history, they confirmed they have never been confined in a detention facility. Jordan does not have any children, so those sections remain blank. We finished the paperwork around 2:00 PM and Jordan is feeling optimistic about the upcoming semester.
