Subject: Case Review Summary for the Miller Family

I have completed the periodic assessment for the Miller family residing in the Northern Region. The primary family members are Robert and Elena Miller. We have been tracking the progress of their child, Jason Miller, currently listed under CYCIS I.D. #9928374-X.

During my home visit on Tuesday, it became clear that the family’s current living environment is the primary obstacle preventing Jason’s return home. The parents are working hard, but the apartment is currently cited for severe environmental neglect, primarily due to a lack of safe, functional shelter and inconsistent access to basic nutritional supplies for the child. While there are no other siblings currently in the household, the lack of stability in their living situation is the defining barrier. Consequently, I am certifying the family as a Norman Class member, as their current circumstances fall squarely under the environmental neglect criteria. I have confirmed that the permanency goal is indeed for the child to return home.

I have reviewed the current service plan, and it directly addresses these barriers. My supervisor, Marcus Thorne, is aware of the filing. I expect to have the follow-up inspection scheduled by the end of the month to reassess the shelter conditions. Please process this certification for the Region 4 administrative file. If any further information is required, please reach out to my office directly.
