How IconicCare fills a 15-page credentialing application in under 90 seconds instead of 1–2 hours
IconicCare provides physician-led rehabilitation and medical care to patients in nursing homes, assisted living communities and residential care facilities. The physicians, nurse practitioners and other providers it works with are independent contractors, and each one has to be credentialed with a detailed application. That application existed only as a flat PDF. Instafill.ai converted it into a fillable form, and an application that took 1–2 hours by hand now takes under 90 seconds once set up.

The challenge
The form is the Haven Health Independent Contractor Initial Credentialing Application, a 15-page packet that asks for:
- Personal information, including SSN, NPI and Medicare and Medicaid numbers
- Education and training history, with month and year dates
- Board certifications, state licenses with expiration dates, and DEA and controlled substance registrations
- Work history for the past 10 years and hospital affiliations in reverse chronological order
- Professional references, 19 yes/no disciplinary questions and malpractice claims history
- Authorization, attestation and release signatures
The PDF had no interactive fields at all. Staff printed it and filled it in by hand, or added text boxes one field at a time in a PDF annotation tool. Every application started from scratch, even when the provider's information was already on file, and copying license numbers, dates and certifications from spreadsheets and emails into 15 pages invited mistakes. Without fillable fields, there was no way to connect the form to any form-filling tool or to the data IconicCare already had.
The solution
IconicCare subscribed to Instafill.ai and uploaded the flat PDF. Within a few hours, flat-to-fillable conversion had turned it into a fillable form, with each field's type (text, date, checkbox or signature) detected and named, and it was ready to use in IconicCare's account.
To fill an application, IconicCare now uploads the provider's CV, license documents or data files, and filling from documents maps what they contain to the right fields. Dates come out in one format, gaps in work history are pointed out, and incomplete sections are flagged for review.
A provider's details can be kept in a reusable profile for renewals and for applications to other facilities, and when several providers start at once, batch filling from a spreadsheet completes all their applications in one run.
The same conversion works for any other flat form IconicCare receives, so it no longer depends on a hospital, payer or medical group sending a fillable one.
The results
| Before | With Instafill.ai | |
|---|---|---|
| Time per application | 1–2 hours | Under 90 seconds after setup |
| The flat PDF | Printed and handwritten, or annotated field by field | Converted once into a fillable form |
| Provider data | Retyped from spreadsheets and emails | Read from CVs, licenses and data files |
| Several new providers | One application at a time | Batch-filled from a spreadsheet |
What used to mean printing, handwriting, scanning or annotating a PDF field by field is now a fill and a review, with less manual data entry and less risk of transcription errors. IconicCare onboards its independent contractors faster, and any new form that arrives flat can be converted the same way.
