How a GP practice fills 100–300 DVA D9287 Diagnosis Forms a month from one CSV
Med For Men is a men's health GP practice in Brisbane that prepares medico-legal paperwork for veterans claiming compensation from Australia's Department of Veterans' Affairs (DVA). Every condition a veteran claims needs its own D9287 Diagnosis Form. With Instafill.ai, a veteran's whole set is filled from one CSV, merged into one PDF and signed in a single pass.

The challenge
A typical claim has 7–10 diagnoses, and the practice sees 15–30 veterans a month: 100–300 forms. Each was typed by hand, signed by the doctor and combined into one PDF for DVA. Combining them in Acrobat caused its own problem: the forms share field names, so the first form's answers appeared on every form after it.
The solution
Each veteran's diagnoses go into one CSV, one row per diagnosis: patient, diagnosis, basis, date of onset and date of first consultation. Batch filling turns every row into a completed D9287, and the practice's details are typed once for the whole batch.
The filled forms are then merged and flattened into one PDF, one diagnosis per page. The merged pages no longer carry form fields, so no answer can leak from one form into the next.
The doctor signs every page of the merged PDF in Instafill.ai's viewer, with no separate signing tool. With automatic data cleanup set to 1–2 days, the CSV, the inputs and the filled forms are deleted soon after the set goes to DVA.
The results
It's working perfectly now. Everything fills and merges exactly the way we need.

The practice handles its full volume, 15–30 veterans and 100–300 D9287 forms a month, and a veteran's complete set takes minutes instead of hours. Filling, merging, signing and deleting all happen in one place, where before they took Acrobat, a separate signing tool and manual cleanup.
The D9287 Diagnosis Form

The D9287 is the DVA form a medical practitioner completes for each condition in a veteran's compensation claim. One form covers one diagnosis.
It asks for the patient's name and date of birth, the medical diagnosis and the basis for it, related conditions, the date of onset and of first consultation, and the practice's address, phone, provider stamp and signature.
