How a Las Vegas veterans benefits service fills 15+ VA DBQs without retyping provider credentials
VA Disability Increase, based in Las Vegas, Nevada, helps veterans file disability claims, appeal denied claims and pursue increased ratings. Every claim needs Disability Benefits Questionnaires (DBQs), the VA's standardized medical forms, and the company keeps a library of 15+ DBQs with its provider information already filled in. With prepopulated fields in Instafill.ai, those credentials stay on every form and only the veteran's information is filled.

The challenge
The company gathers medical information in consultations with veterans and turns it into completed DBQs. Its library covers musculoskeletal conditions (ankle, back, foot, hip, knee, lumbar spine, neck, shoulder, wrist), neurological ones (bilateral upper extremity nerves, migraine, peripheral nerves) and others, including erectile dysfunction, GERD, hypertension, obstructive sleep apnea and rhinitis.
Every one of those forms carries the same provider block: the examiner's name and credentials, area of practice or specialty, contact information, National Provider Identifier (NPI), medical license number and state, and the examiner's address. Standard form automation clears whatever is already in a form when it fills new information, so the pre-filled credentials would have been wiped on every fill. And because VA ratings set a veteran's monthly compensation, an error or inconsistency in a DBQ can delay a rating or affect it.
The company needed the provider credentials kept, the veteran's information filled automatically, the same approach on all its DBQs, and consistent details across every form for one veteran.
The solution
Instafill.ai built prepopulated static fields for this request. VA Disability Increase uploaded its DBQs, filled in the provider credential fields once and saved each form. Every field holding a value at that point becomes static: it is kept, unchanged, in every future fill of that form.
Only the empty fields are filled for each veteran: name and contact information, service dates and military history, symptom descriptions, medical history and the condition-specific clinical information. Each completed DBQ combines the provider's saved credentials with that veteran's own medical information.
Keeping the library current is the same step. When the provider information changes, the form is updated and saved again, and new DBQs are added as the VA releases updates. The guide to prepopulating PDF form fields with static data shows the setup.
The results
| Before | With Instafill.ai | |
|---|---|---|
| Provider credentials | Entered by hand on each form | Saved once per form, kept on every fill |
| Consistency | Re-entry could differ between forms | Identical on every form, from one source |
| Staff time | Spent retyping credentials | Spent documenting symptoms and findings |
When a veteran's claim needs several DBQs, all of them go through one workflow with the same provider information across the whole package. Automatically kept credentials remove one source of error from forms that decide a veteran's compensation.
The Sleep Apnea DBQ

The Sleep Apnea DBQ is one of the questionnaires in VA Disability Increase's library. It is meant to be completed by the veteran's healthcare provider and runs to four pages: diagnosis, medical history, findings, other physical findings, diagnostic testing, functional impact and remarks.
Section VIII, the examiner's certification and signature, holds the provider fields the company saves once: the examiner's printed name and title, area of practice, phone and fax numbers, NPI, medical license number and state, and address.
