Check this box if you want an Account Transcript showing account activity such as payments, penalties, and adjustments after the return was filed (for the tax form number entered on line 6).
Record of Account Checkbox
Check this box if you want a Record of Account transcript that combines the Return Transcript and the Account Transcript (for the tax form number entered on line 6).
Verification of Nonfiling Checkbox
Check this box if you need proof from the IRS that you did not file a tax return for the year requested.
W-2/1099/1098/5498 Transcript Checkbox
Check this box if you want a transcript of information returns (such as Form W-2, Form 1099 series, Form 1098 series, or Form 5498 series) reported to the IRS for the year requested.
