Check the box next to the appropriate option (No – Neither, Yes – Aboriginal, Yes – Torres Strait Islander, or Yes – Both) to indicate the client's Indigenous identification.

Check this box when the client did not state their Indigenous status or their response was insufficiently detailed to categorise.

Enter the client's gender identity (for example Male, Female, Non-binary or other self‑described term).

Enter the client's ethnicity or cultural background (for example Australian, Anglo‑Celtic, Asian, Aboriginal and Torres Strait Islander, etc.).

Enter the client's current marital status (for example Single, Married, De facto, Separated, Divorced, Widowed).

Enter the client's preferred language for communication (for example English, Mandarin, Arabic), including dialect if relevant.

Enter the country where the client was born using the full country name (for example Australia, New Zealand).

Enter the client's given first name(s) as you want them recorded.

Enter the client's family/surname.

Enter the client's Aged Care ID assigned by My Aged Care or the service (include any letters or digits exactly as shown).

Enter the client's date of birth.

Enter the client's Medicare card number exactly as shown on the card, including any reference numbers or letters.

Enter the client's Department of Veterans' Affairs (DVA) number if they have one.

Enter the client's primary email address for contact.

Enter the client's mobile phone number, including country or area code if relevant.

Enter an alternative phone number for the client (for example home or work), including area code if relevant.

Specify the type of address provided (for example residential, postal, temporary or other).

Enter the client's full address, including street, suburb, state/territory and postcode.

Enter any additional comments, observations, or relevant information about the client that clarify or supplement the form (e.g., special circumstances, notes, or explanations).

Check this box if the client's communication difficulty is cognitive in nature (for example problems understanding, processing, remembering, or organizing information). Fill only if 'Communication help needed — Yes' is 'Yes'.

Check this box if the client has a hearing-related communication difficulty that affects their ability to hear or follow spoken communication. Fill only if 'Communication help needed — Yes' is 'Yes'.

Check this box if the client has language-related difficulties (problems understanding or using words, sentences, or language structure). Fill only if 'Communication help needed — Yes' is 'Yes'.

Check this box if the client has speech-related difficulties affecting articulation, clarity, voice, or the physical production of spoken sounds. Fill only if 'Communication help needed — Yes' is 'Yes'.

Check this box if the client has a different type of communication difficulty not listed above and provide details in the comments/information section. Fill only if 'Communication help needed — Yes' is 'Yes'.

Check this box if the client ever needs help to communicate (to understand others or to be understood by others).

Check this box if the client does not need help to communicate and can understand and be understood by others without assistance.

Check this box if the client holds a DVA White Card entitlement.

Check this box if the client holds a DVA Gold Card entitlement.

Check this box if the client holds a DVA Orange Card entitlement.

Check this box if DVA entitlement is not applicable or the client does not hold any DVA card.
