Client File: Leo R. Thompson (ID: 88293-LT), currently 9 years old (DOB: 03/12/2016). Leo remains in a therapeutic foster home placement, receiving a monthly maintenance stipend of $1,250. During my recent home visit, I noted that while Leo is adjusting, his current medication regimen for ADHD needs oversight. He is seen by Dr. Aris Thorne at Mercy Pediatrics every three months for follow-ups; the last visit was January 10, 2026. Medications include Adderall XR and Guanfacine. Notably, Leo was hospitalized briefly last November for a severe asthma flare-up; he spent 3 days at Springfield Memorial and was discharged on November 15, 2025. 

Regarding educational support, Leo is currently in the 3rd grade and receives special education services under an IEP, most recently dated September 22, 2025. The school has requested a new MDE conference to address his current behavioral struggles. Unfortunately, the current foster parents have expressed difficulty in coordinating consistent transport to his behavioral therapy sessions, which is creating a service delivery gap. We have identified several unmet needs: first, professional behavioral counseling is needed to address recent outbursts (to be addressed by Regional Behavioral Health by March 2026); second, the child requires an updated occupational therapy assessment (to be addressed by the school district by April 2026); and third, we need a formal mentoring partner to support his social transition (to be coordinated by the volunteer services department by May 2026).
