Caseworker Notes - June 12, 2026. Regarding the placement of Leo Marcus Thompson (ID: 88293-LT), currently residing in our traditional foster home site under the Sunshine Family Agency (Region 4). Leo, born on January 14, 2017, has been under the supervision of caseworker Sarah Jenkins (555-0199). During our recent review, it was noted that Leo’s behavioral management plan has evolved. He now requires significant daily administration of his behavior modification plan by the foster parents, which they are handling well following the training sessions. His mental health needs are currently classified as 'Moderate' due to his inability to tolerate unstructured time; consequently, he requires increased direct supervision and structuring of all daily activities to mitigate his tendency for acting out. He attends therapy sessions weekly, which necessitates the foster parents' participation in the clinical process at least three times a month. Academically, Leo is in a self-contained classroom under his current IEP, necessitating frequent interaction between his teachers and the foster family. His last recorded LOC score was 64. Given his progress and stable medication regimen, we have determined that he is not exempt from annual reviews, and I recommend a re-review period of 6 months. His current foster home is fully licensed. The total LOC score calculation based on these observations puts him in the Treatment Foster Care category. Please update the master file and process the effective date as July 1, 2026.
